#7) Fat Brain

Weight loss clients are fun, interesting to say the least and a doctor has to be on his toes as these folks can be very elusive.  This is what I hear frequently in my Bellevue clinic, "Dr. Doyle, I want to lose the weight, I am tired of looking and feeling fat and wearing fat clothes and if I don't lose the weight now , I just know that in 10 years I will be a lot bigger."

Now as you  read these words you  are probably  thinking, "what is wrong with that, sounds to me like a cooperative and eager client?"  Yeah,  but she is not finished yet.  Then with great anxiety she explains, "I don't really know about this weight loss stuff, I have tried so many different methods and nothing works, actually my entire family is big and I think I am just big boned."   I then review again with her the examination results we just obtained from her body composition analysis.   "At 5' 3" tall you weigh  160 lbs.with a body fat of 44% (less than 28% advised) and a muscle mass of 21% (28% advised)  and 12 rating (3-5 rating advised) of visceral or belly fat. You should weigh around  120 lbs. and lose 40 lbs. of fat.  You are 55 years old yet your body age (metabolic age) is 72 years.  You have high blood pressure, high cholesterol, your gall bladder removed, indigestion, poor sleep, no energy and frequent bouts of sadness." "Well,  Dr. Doyle, you know, no one lives for ever, skinny people die too you know."
"Dr. Doyle , I really do want to lose weight but I don't want to give up my baking and my afternoon wine and cheese, my hubby and I love our afternoon wine and cheese, we really look forward to that time."  And then I explain to her that she is not a good candidate for the program as I now recognize the need to put a stop to her excuses and denials and self sabotage.

Who I am dealing with right now is  her "Fat brain", her addiction to the junk she consumes and refuses to let go.  In reality I have been talking to this addiction since the moment she sat down for consultation with me, this person is addicted to her daily fix of  sugar in the form of bakery goods, breads, pasta's  and alcohol, she is addicted to all the junk that has made her fat and sick and miserable.  She also has  lifestyle habits and behaviors she does not want to change.  So I tell her this truth and then she impulsively stands up out of her chair, bends forward toward me, looks me straight in the eye and tells me with absolute conviction, "I am ready to change Dr. Doyle, I need to lose this weight."

At this point I am a bit confused as to who I am dealing with here.  Is it the person who had just explained to me that skinny people die and that she does not want to give up her baking and her wine and cheese?  Or is it the person who just emphatically told me she is ready to change?  I tell her that  I want her to go home tonight and wrestle with the devil.  I want her to battle it out with that aspect of herself that wants to slim down and transform and the other addicted aspect that wants her to continue with the status quo, there can only be one winner. If  your addicted fat brain wins then you go about your life and you change nothing.  If the real you wins this battle of commitment and desire for change,  then you come back tomorrow a victor and you come back resigned to changing your habits  letting go the garbage and the junk in your diet.  If you do return, it must be with the goal and objective of  weight loss for life, not just dropping some pounds or hitting a target weight loss goal.  Go home and make your final decision, if I do not see you tomorrow then I will know who won but if I do see you again I will know you are 100% committed to not just losing the weight but changing your life for the better.  I gave her a hug and she left my office with a big decision to be made that night.

Three days later she returns, she was relaxed and at peace with her now decision, she even looked slimmer to me, she had been cutting back dramatically on her addictive food intake. She had no excuses  for me, no denials, no ultimatums on what she could and would not do on the program,  essentially her fat brain was disconnected, no longer in control.  There was no arguing, no disagreements, no requests or demands by her to do it her way.

She has now completed week three of a 12 week program, she lost 5 lbs first week, 7 lbs second week and 4 lbs her third week.  She has no hunger, feels better than ever, has much more energy and no more sad moods, she has lost lots of inches noticeable in how her clothes fit.   I have no concern that she  will  not achieve weight loss for life, the fat brain  has been replaced with a confident, energetic, empowered woman with  a renewed purpose and an energetic zest for life.
